Informed Consent for Herbal support from Herbalist & Naturopath Laura Grigaliute T/A Herbstore & Herbalist.
Herbal med and Naturopathy is practiced by non-medical professionals and is complementary to other regulated forms of health care in Ireland. Consultations with a Herbalist and Naturopath include taking a case history, creating individualised plan and herbal formula, and following up on results and progression of your health. Herbalist may employ a range of techniques including dietary advise, supplements and lifestyle advise.
While the best course of action is continually sought for the client there always exists the possibility of side effects and adverse reactions Herbalist & Naturopath Laura Grigaliute holds your safety and well-being as her top priority in the management of your case and welcomes all questions or concerns you may have.
By agreeing to the terms of engagement you acknowledge that:
1. Herbalist & Naturopath Laura Grigaliute has in no way suggested that you being under her care should prevent you from seeking help from any other healthcare practitioner.
2. Herbalist & Naturopath Laura Grigaliute will strive to deliver the safest and most effective herbs for your case, however there is still the possibility that side effects or adverse reactions might occur, or that benefits may not be achieved.
3. You will inform Herbalist & Naturopath Laura Grigaliute of all medical conditions you have been diagnosed with, symptoms you are experiencing, and medications you are taking/have taken in the past. You will also inform her of any new medical conditions or symptoms or medications should they arise.
4. You will inform Herbalist & Naturopath Laura Grigaliute if you are pregnant or breastfeeding. You will immediately inform her should you become, or plan to become pregnant or if you begin, or plan to begin to breastfeed.
5. You will inform Herbalist & Naturopath Laura Grigaliute if you do not understand any given part of the recommendations given or if you are uncomfortable with any aspect of your care.
6. All of the information you provide to Herbalist & Naturopath Laura Grigaliute is protected by General Data Protection Regulations and is confidential unless disclosure is required by law.
7. Your case information may be used for the publication of case reports or case studies. Any information concerning your identity will be excluded from publication, thus maintaining your anonymity.
8. You are free to purchase any products recommended by Herbalist & Naturopath Laura Grigaliute from a vendor of your choosing, being under no obligation to purchase products from Herbalist & Naturopath Laura Grigaliute directly.
